Patient Registration

We are happy to register patients who live within our practice boundary. For information on whether you live within the boundary, please contact the reception staff who will be happy to assist with your query or click on the link below.

To register, please download, print and complete the form below, for each family member. Alternatively, please come into the practice and we will provide you with a paper copy of the forms. We require you to also complete our New Patient Questionnaire which can also be accessed using the under noted link.

Please notify the practice of any change in contact details so that we can keep your records up-to-date.

If you relocate outwith the practice boundary it may be necessary for you to register with a new practice.

Patient Confidentiality

Your Medical Records are confidential. Under no circumstances is information about you shared with third parties who do not directly contribute to or support the delivery and planning of your health care, unless your consent has been obtained.

Suggestions and Complaints

We strive to provide a quality service to our patients. We welcome any suggestions or comments to improve this service.

If you feel that your needs are not being met, we would welcome the opportunity to discuss this with you.
